Membership Change Options Explained

There are 4 types of Membership Changes to choose from. If you are not sure which option is the best for you, please read this entire section before filling out this request form.

 

Training Break *Recommended for most situations*

  • Take a leave of absence for ANY reason for 1 week or longer.

  • During a Training Break, you continue paying every month according to your membership agreement

  • You get Time Credit on to the end of your program, so that you get back all the time you paid for.

  • We can only give Time Credit to you if you've give us at least a weeks advanced notice through the form below and specify both a leaving and returning date.

 

Medical Billing Hold

  • A Medical Billing Hold is when you stop paying and stop training for a month or longer due to physical disability.

  • There is no monthly fee for this hold, but a doctor's note and a week's notice is mandatory to start this.

  • You are still responsible for the remaining time left on your agreement after you return.

  • All Medical Holds need to include a return date within 6 months. It can be changed at any time with another's doctor's note.

  • Any Medical Hold that does not have a doctor's note attached will be treated as a Premium Billing Hold.

 

Premium Billing Hold

  • Pay a monthly fee to pause your regular Tuition for a month or longer.

  • Enroll in this for any reason with notice of a week or more.

  • Pay only $25/month for an individual or $50/month for a family until you are ready to return to your training program.

  • Secure your tuition rate, and get unlimited Zoom training while you are away. We'll save your spot on the mat and continue to follow up with you to get you back in.

  • There is an agreement to sign for this type of hold. You'll be forwarded to sign up and pay after filling out this form. No hold will start until payment and agreement are received.

 

Cancellation

  • You can cancel your agreement early for any reason, but we kindly ask that you describe your reason for cancelling in the comment section of our form.

  • There is a standard cancellation fee of $195 for an individual or $390 for a family unless your agreement says otherwise.

  • We need 30 days notice for any Cancellation unless otherwise stated in your agreement. This means all bills within the 30 days will be collected in addition to any cancellation fee.
